Overview

Circare is a not-for-profit community based mental health agency located in Syracuse, New York. Circare programs are designed to help individuals and families, who have encountered certain obstacles develop the resources and supports to live a satisfying, naturally independent life.

 

The Circare Intake Program Manager will interface and collaborate with Circare departments, Health Homes Networks (HHUNY, CHUNY, Encompass), Community providers and Managed Care Organizations (MCO) to provide oversight and direction to ensure that Health Home referrals follow Department of Health (DOH) Regulatory Guidelines as well as Circare best practices.  The Program Specialist will provide leadership and intake training by participating and facilitating in Circare Department meetings, community provider meetings, and pilots and projects as assigned. The Program Specialist will be responsible for interfacing with interdepartmental staff to provide relevant data and outcomes in relation to enrollment processes for Circare Departments

 

Pay Rate: $25.48/hr.

 

Responsibilities

  • Interface and work collaboratively with Health Homes Networks (HHUNY, CHUNY, Encompass), Managed Care
  • Organizations (MCOs), internal Circare departments, community providers and referral sources (with appropriate provisions to ensure privacy is protected) and other providers as assigned to manage the electronic referral and assignment process to Health Home Care Management Services and/or HCBS Services.
  • Assist and provide intake and outreach education and training oversight to CMs, Supervisors, Outreach and Engagement
  • Team, and new hires to ensure rapid intake and enrollment processes are completed to regulatory and compliance standards.
  • Coordinate and Analyze data outcome measures to interface and collaborate with interagency departments, MCOs, Health
  • Homes Networks (HHUNY, CHUNY, Encompass), DOH and other community stakeholders.
  • Develop and provide data tracking trends and outcome measure using HIT.
  • Supervise Referral Specialist regarding Outreach engagement training.
  • Co-facilitates and prepares for referral meetings.
  • Attend supervisory meetings and weekly leadership management meetings.
  • Coordinate walk in schedule for intake.
  • Provide coverage as needed.
  • Supervise new hires and referral specialist position around Outreach engagement duties.
  • Complete weekly audits to ensure outreach and engagement activities are being conducted.
  • Review client charts prior to transfer for quality measures.
  • Meet with staff in a group and or individual format based on program need to provide instruction and oversight of outreach and engagement activities.
  • Coordinate with Coordinator of Outreach and Engagement regarding capacity of HUNY referrals-determine number of referrals allowed each month.
  • Operate new hire training around care manager practices coordinate this with Coordinator of Outreach and Engagement.
  • Develops and provides internal tracking and status dispositions for enrollment, transfers, and discharges.  Secures relevant eligibility documentation
  • Other duties as assigned.

Qualifications

  • Master’s degree in one of the qualifying fields* and one (1) year of Experience; OR
  • Bachelor’s degree in one of the qualifying fields* and two (2) years of Experience; OR
  • A Credentialed Alcoholism and Substance Abuse Counselor (CASAC) and two (2) years of Experience; OR
  • Bachelor’s degree or higher in ANY field with either: three (3) years of Experience, or two (2) years of experience as a Health Home care manager serving the SMI or SED population.

 

Qualifying fields include education degrees featuring a major or concentration in social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing or another human services field.

 

Experience must consist of:

 

Providing direct services to people with Serious Mental Illness, developmental disabilities, alcoholism, or substance abuse, and/or children with SED;

OR

Linking individuals with Serious Mental Illness, children with SED, developmental disabilities, and/or alcoholism or substance abuse to a broad range of services essential to successful living in a community setting (e.g., medical, psychiatric, social, educational, legal, housing and financial services).
